Central New York Care Collaborative Connects Healthcare Partners Across Six Counties

Central New York Care Collaborative (CNY Cares) is a partnership that connects more than 2,000 local healthcare and community based service providers across six counties in Cayuga, Lewis, Madison, Oneida, Onondaga, and Oswego. Their goal is simple, working together to provide better healthcare.

They are supported and funded by the New York State Department of Health and their primary focus is to try to improve services and the overall health of the Medicaid community in the six county region. They work with everyone from hospitals, nursing homes, behavioral health clinics, pharmacies, community based organizations such as the Salvation Army, and so much more. They then utilize that network to build services around that Medicaid community.

Director of Communications BJ Adigun shares some examples of how CNY Cares is working with partners to provide incentives to help develop some projects and programs to support the community. One project is an emergency department navigator that is partnered with 11 area hospitals. Anyone who has visited an emergency room has likely experienced a wait time. CNY Cares is hoping to shorten those waits by sending individuals that use an emergency department for something such as an ear infection to another location. If these individuals are connected with a primary care practice in their neighborhood, hopefully they will not return to the emergency room again with a non-urgent condition.
